Winner: RDaSH Community Nursery Nurses & Family Hub Team – Delivery of community First & Growing groups.
Runners up;
EPIC: Team Around the Street - Doncaster Children's Services Trust in collaboration with South Yorkshire Police, preventing young people entering the criminal justice system.
Andrew Maddison – Public Health & School Nursing working in partnership to improve delivery of the service specification.
- Health & Wellbeing Champion (Young Person): "This award recognises outstanding achievement of a young health champion in raising a health issue amongst their peers & making an impact on their communities health & wellbeing."
Winner: Alex Winter, Young Commissioning Programme - support in the commissioning of services for young people of Doncaster.
- Health & Wellbeing Champion (adult): "This award recognises outstanding achievement of an adult champion in going the extra mile to support the health and wellbeing of young people in Doncaster."
Winner: Lauren Watts, Woodfield Primary School – supporting numerous public health projects at Woodfield Primary School.
Runners up:
Bex Shaw, Doncaster Green Team - working tirelessly in the community where she lives to make the environment as attractive as possible for people to live & children to grow up in.
Michelle Boardman, Junior Youth Club Hexthorpe – weekly youth drop in at St Judes church hall for all in the Hexthorpe area.
- Innovation: "This award showcases innovation to improve the health & wellbeing of young people"
Winner: Sophie Uzelac - Health Visitor & Leana Gater - Nursery Nurse – Documentary of weaning onto solid foods by Sophie & daughter Anna.
Runners up:
Family Moving On Together (FMOT) programme, RDaSH Project 3, RDaSH/ADS Aspire and YWCA Green Gables (Stronger Families) – A whole family programme aimed at supporting families impacted by substance misuse.
Gavin Hinchliffe and Sarah Crossland , Doncaster College – raising awareness of health & wellbeing related topics via the online curriculum.
- Engagement with children and young people: "This award demonstrates actively seeking views from young people, which is then used to shape and influence practice."
Winner: Health Improvement Team, School Nursing – for work with Early Help & supporting children, young people & their families who's needs may be more complex.
Runners up:
Theressa Stothard, Junior Citizen Programme - A programme delivered to 80 children/week in 1 hour sessions over the academic year.
The Ladder Group – a group made up of young people with special educational needs and/or disabilities.
- Team: "This award recognises a team that goes above & beyond to support the health & wellbeing of children & young people."
Winner: West Road Primary School Admin team – nominated for always going above & beyond for every child, staff member & parent.
Runners up:
Doncaster Council – The Junior Citizen Programme a multi-agency approach delivered in Edlington.
Doncaster College Welfare Team - Working over & above their remit making sure vulnerable young people can access an education.
- Community health & wellbeing project: "This award celebrates the success of a place based initiative that improves the health & wellbeing of children & young people in a way that contributes to long lasting change."
Winner: Stainforth family hub parent voice - changes in the families lives in the family hub & the community.

Little Einsteins Day Nursery Wheatley – participation in supervised toothbrushing scheme & the Healthy Learning, Healthy Lives programme.

Winner: Barnby Dun Primary Academy – for work towards becoming an eco-school.

Coppice Special School – commitment to staff wellbeing.
Rowena Academy – healthy fundraising activities for the schools chosen charity - Children’s Air Ambulance.

Winner: The Levett School – work of the staff supporting young people & their families.
Runners up:
Stone Hill Special Needs School–work with the b:friend charity & The Prince’s Trust community project.
Maple Medical Centre–commitment to health & wellbeing, introduction of ‘Maple Mile’ & engagement with HLHL.
